Everyone in Webb County qualifies for services at the Center. To register for services, a client must bring proof of residence, e.g. a recent light bill, a personal photo ID, or documentation that he/she has Medicaid/Medicare or private insurance.  If a client chooses to apply for a discount, he/she must provide a current family income tax return or check stubs, social security number, and birth certificate for each member he/she wants to enroll.  The Center uses the Federal Poverty Income level guidelines to determine the discount found below. The guidelines are based on income and family size. The registration office will issue clients a card that will indicate the discount category he/she falls into and will also list each member of the client’s family. If the client does not provide proper documentation, he/she will be charged 100% of cash for all services rendered.

All residents of Webb County qualify for services

TO REGISTER OR TO UPDATE FOR SERVICES, PLEASE PROVIDE:

1. Current Income Tax Return including W-2 Forms for all working family members.

    If you did not file an Income Tax Return, please bring four of your most current check stubs;

 

2. Light bill which must be under the name of the person applying for services

    If not available, two other utility bills will be accepted.

    If none are available, please contact the registration office for further instructions;

 

3. Medicare/Medicaid, Private Insurance or any other third party coverage;

 

4. Copy of Social Security/Supplemental/Veterans or Pension checks;

 

5. Birth Certificates or Baptismal Certificate with parents name for children under 18 years of age and younger;

 

6. Social Security Cards for all family members, if available (copy not required);

 

7. Immunization records/cards for all minors.
