PATIENT EDUCATION

Diabetes Self-Management Courses
Clients attend once a week sessions for 10 consecutive weeks to learn about the disease and how to control it. Classes include information on healthy eating, physical activity, the health care team, complications, and family support.

Weight Management Program
The program is for children and their parents, and consists of a 10-week course (once-a-week-meetings). The program provides information on nutrition, exercise, and active living.

Support Groups
Support groups for people diagnosed with Diabetes/ Cardiovascular Disease (DM/CVD) are designed to provide emotional and social support. The two hours sessions are a combination of review of self management goals, hands on activities, inspirational segment, and sharing personal experiences.

Breast and Cervical Cancer Prevention Services
Clients attend once a week sessions for 10 consecutive weeks to learn about the prevention and early detection of cancer.

Worksite
Health Promotion Health fairs, educational presentation, health screenings, and disease management activities are conducted at worksites. These activities may be conducted as part of a wellness program.

• Glaucoma Screenings: Screenings are provided free of charge throughout the community to identify potential eye problems.

SERVICES

Diabetic Supplies
Eligible clients may receive glucometers, strips, lancets and solutions at a discounted and affordable price.

Eye Screening Program
Center pays two thirds for a yearly eye exam and up to 80% for eye surgeries for eligible clients.

Footwear

Nutritional Services
Nutritional services are provided by a Registered Dietitian (RD). Services include one to one nutritional counseling and meal plans based on individual needs.
