SLIDING FEE DISCOUNT

We are a federally funded organization. Clients are expected to pay for services rendered; however, a client will not be denied an office visit because of his/her inability to pay. The Center provides a schedule of discounts to all individuals and families with an annual income at or below 200 percent of the most recent U.S. Department of Health and Human Services Federal Poverty Guidelines. FEES/BILLING INFORMATION
The amount of a client’s bill will vary depending upon the type(s) of service(s) he/she receives. Note: The Center will arrange a payment plan should the client not have the money required for services.

For clients who fall below 100% federal poverty level income, a minimum user fee of $15 per office visit will be required in addition to any applicable fee(s) for any ancillary service(s) provided.

For Medicare clients, the Center will waive the $200 deductible and 20% co-pay for office visits. However, Medicare clients will be required to pre-pay for medications at the rates mentioned on the previous page.

All moneys collected from clients for outpatient, inpatient, and urgent care services go back into the Center’s operating budget and are used primarily to offset the high costs of medication, laboratory, and X-ray services. Clients’ contributions are essential in maintaining the accessibility and delivery of quality services.

NOTE: All fees subject to change. Billing fee scales for our Urgent Care Clinic and hospital services differ from above.
