Explanation of Benefits
PAID LEAVE:
Annual Leave - Vacation
Full-time and Part-time employees accrue annual leave on the first day an employee is physically on duty and ends on the last day he/she leaves employment with the Center. Employees working any part of the month accrue leave for the full month.
Note: No leave may be taken before actual leave is earned. Unused annual leave days are paid out upon termination of employment on the next payroll period following the last day worked provided the employee has successfully completed his/her introductory period.
Sick Leave
Full-time and Part-time employees accrue sick leave at the rate of 8 hours per month for full-time and 4 hours for part-time employment. Sick leave is accrued the first day of work. Unused sick leave days are NOT paid out upon termination of employment.
Note: Sick leave may be taken during introductory period.
Holidays
Ten (10) days.
INSURANCES:
Health (BCBS)**
The Center offers several options for health insurance to its employees, with varying levels of premiums. Please consult with Human Resources for specifics on the current options available.
Dental (Ameritas)**
Dental at no cost to the employee. ($50.00 deductible waived on preventive service; exams, cleanings and x-rays)
Life Insurance (BCBS)
Life Insurance policy of $15,000 at no cost to the employee.

RETIREMENT PLANS
All employees who are 18 years old and completed their introductory period are eligible for the Center retirement plan. For employees who participate in the plan, the Center will contribute an amount equal to 4% of the employee’s salary up to a maximum of $1,000 per year. Employees do not contribute to this plan.
Plan 403b – Employees can contribute up to $16,500 per year tax deferred. Employees over age 50 may contribute an additional $5,000. (Some employees may have an additional tax savings under the IRS rules for Earned Income Tax Credit. Please consult with your financial advisor.)
Plan 457 – This retirement plan is for Providers/Executives earning over $100,000 a year.
NOTE: HEALTH, DENTAL, LIFE, AND RETIREMENT PLANS ARE AVAILABLE AFTER THE EMPLOYEE HAS SUCCESSFULLY COMPLETED HIS/HER INTRODUCTORY PERIOD.
