Gateway Community Health Center, Inc. is a federally funded 501(c) (3) private, not for profit corporation. Everyone qualifies for medical and dental services at the Center. To register for services, a client must bring proof of residence, e.g., a recent light bill, a personal photo ID, or documentation that he/she has Medicaid/Medicare or private insurance. If a client chooses to apply for a discount, they must provide a current family income tax return or check stubs, social security number, and birth certificate for each family member they want to enroll.
The Center uses the Federal Poverty Income level guidelines to determine the discount the client will receive based on income and family . If the client does not bring proper documentation, they will be charged 100% of charges for all services rendered. The registration department will issue the client a card that lists each member of the client's family and indicates the discount category the client falls into.
